NTSB report details how controller decision and tech failures led to deadly runway collision

At LaGuardia, a controller cleared an emergency truck onto an active runway as a plane descended at 130 feet. The warning came too late. The runway lights even went dark 3 seconds before impact. Both pilots were killed because every safety net failed.
